"Return item from customs in origin country" is a Pakistan Post scan made when your parcel comes back from export customs in Pakistan. "Return" here means the parcel has been handed back to the postal operator after customs processing. It does not mean it is being sent back to the sender. The parcel is still in Pakistan, at or near the international mail exchange office, and has cleared the export check.
By itself, this message is normal. It is a routine step for international parcels leaving Pakistan, and it usually follows the scan recording that the item was sent to customs. If the parcel stays on this status for longer than the previous steps took, that is worth watching, but the message alone does not point to a problem.
