UPS tried to deliver the parcel and the person receiving it declined to accept it. The parcel is still with UPS, usually on the delivery vehicle or back at a local facility. Most often this line follows "Out For Delivery Today", so the refusal happened at the door or was recorded during the delivery attempt.
This is an exception, not a normal delivery step. The parcel will not be handed over to you as planned. If you did not refuse it yourself, the refusal may have been recorded by mistake or by someone else at your address. Treat it as a problem worth checking.
UPS usually repeats the refusal in later updates and says it will contact the sender for instructions. In some cases the next entry says the parcel is being returned to the sender. Less often, there is a further scan at a warehouse or a note that the address was corrected or the receiver was contacted. The sender decides what happens to the parcel, and the return trip can take longer than the earlier steps.
