UPS is telling you that a train derailment has disrupted a rail leg of your parcel's route. The parcel is somewhere between UPS facilities and is moving by the network, but the transport it was planned on is blocked or slowed. UPS is rerouting it, which is why the message says it is adjusting plans.
Yes, this message is an exception. It is a delay notice, not a sign that the parcel is lost or damaged. The derailment affects the route, not your parcel in particular. Delivery will probably be later than first planned, but UPS does not give a new date in this message.
The parcel usually continues through normal facility scans. In about half of cases the next update is another "Arrived at Facility" entry marked as "On the Way", and in about a third it is a plain "Arrived at Facility". A smaller share show "Departed from Facility". Once the parcel reaches a facility and is sorted onto a new route, tracking should return to the normal pattern.
